The Northamptonshire Primary Care Training Hub has been able secure a fully funded CARE Programme for the region. The programme is run in partnership with the National Association of Primary Care (NAPC), NHS England and NHS Improvement, and Bedfordshire, Luton and Milton Keynes (BLMK) Integrated Care System.
There will be a minimum of 25 funded places. Applications are open now and are open to anyone working in Primary Care and can be started from this web page
CARE’s practical and person-centered approach has already led to more joined-up working in primary care networks in BLMK, bringing together a range of health and social care professionals as a complete care community. This is not only helping with immediate acute care need but is also supporting patients whose long-term conditions are likely to deteriorate during the COVID-19 pandemic.
CARE is a learning and development programme that empowers primary care professionals to play a key role in their primary care network, to shape services based on population health needs and to strengthen their leadership. Focussing on wellbeing and building and harnessing resilience, the programme uses digital technology to connect primary care professionals and to support joined-up, multi-professional working.
CARE aims to radically improve population health and reduce health inequalities by connecting and empowering the whole health and care workforce and their local communities to design and deliver improved services for the benefit of patients and staff.
CARE is a holistic change approach led by general practice staff and supports individuals’ resilience and leadership development using a combined focus on:
- • System-wide engagement – connecting national and local teams and strategies.
• Individual activation – using population health improvement projects to anchor learning and demonstrate value.
• Applied leadership – creating and maintaining the right environment for change and innovation.
